What Are the Requirements for Health Insurance for Residency in Spain?

The policy must be 100% comprehensive, with no copays. This means you won’t be asked to pay extra for doctor visits, tests, or treatments. You pay one flat premium — and that’s it.

✔ No Waiting Periods (“Carencias” in Spanish)

Your coverage must be active immediately. Spanish law does not accept policies with waiting periods for services such as hospitalization, surgeries, or specialist visits.

✔ Full Coverage

Your plan must include general medical care, hospital stays, emergency services, surgeries, specialist consultations, diagnostic testing, and more.

✔ Nationwide Validity

The insurance must provide coverage throughout all of Spain — not just in one region or city.

✔ Repatriation Coverage

This ensures that in the unfortunate event of death, the policy covers the repatriation of remains to your home country — including administrative costs and transportation.

✔ Coverage for the Full Duration of Stay

Your insurance must be valid for the entire duration of your residency or visa. If your visa is renewed, your policy must be renewed accordingly.

When Is Health Insurance Required for Foreigners in Spain?

Health insurance is required for a wide range of visa and residency types, including:

  Long-term EU residence

  Registration certificates for EU citizens staying more than 90 days

  Non-lucrative visa applicants

  Investor or Golden Visa holders

  Family member visas for EU citizens
